About the Role:

The Director of Case Management is responsible for implementing and managing the case management program at the local level. This role involves developing systems and processes for care/utilization management and discharge planning. The Director will oversee the department's activities related to discharge planning and clinical quality improvement, ensuring appropriate and effective use of hospital resources.

Position Summary:

The Director of Case Management will lead the case management program, ensuring that hospital resources are used effectively. The role involves overseeing financial and quality data collection, analysis, and reporting related to utilization management, quality improvement, and performance improvement. The Director will promote interdisciplinary collaboration, foster teamwork, and champion service excellence.

Key Responsibilities:

• Care Facilitation : Oversee care facilitation, utilization management, case management, and discharge planning.
• System Development : Develop and implement systems for care/utilization management and discharge planning.
• Financial Oversight : Manage the department's budget, ensuring revenue, expenses, and contribution margins meet or exceed targets.
• Team Leadership : Lead and mentor a high-performing team, fostering a culture of collaboration and excellence.
• Data-Driven Decisions : Use data to drive decisions, plan, and implement performance improvement strategies for case management.
• Education and Communication : Educate physicians, managers, staff, patients, and families about the case management process.

Must-Haves:

• Certifications : One of the following certifications: CCM, ACM, or FAACM.
• Licensing : Current and valid license to practice as a Registered Nurse in the state of Texas.
• Experience : Minimum five (5) years of experience in utilization management, case management, discharge planning, or other cost/quality management programs.
• Nursing Experience : Three (3) years of experience in hospital-based nursing.
• Supervisory Experience : Two (2) years of supervisory experience.

Nice-To-Haves:

• Self-Motivation : Proven communication skills and assertiveness.
• Business Planning : Background in business planning and targeted outcomes.
• Managed Care Knowledge : Working knowledge of managed care, inpatient, outpatient, and the home health continuum, as well as utilization management and case management.
• Performance Improvement : Knowledge of performance improvement concepts.
• Physician Relationships : Demonstrated effective working relationships with physicians.

Minimum and Preferred Qualifications:

• Education : Bachelor's of Nursing (BSN) or Masters Social Work (MSW). A Master's degree is preferred.
• Licenses/Certifications : Current and valid license to practice as a Registered Nurse in the state of Texas or Current and valid Texas license as a Masters Social Worker (LMSW). CCM preferred.
